Parent/Guardian Details
Program Duration: 9 weeks
Session Length: 60 minutes per session
Session Frequency: Once per week
Preferred Class Day & Time (if options are available)
Does your child have any medical conditions, allergies, injuries, or special requirements we should be aware of?
Booking & Payment: I understand that full payment must be made in advance to secure my child’s spot.
Cancellations & Transfers: If my child is unwell, the session will be transferred to the next available class. Cancellations must be made at least 48 hours in advance.
Liability Waiver: I acknowledge that Chakradance involves physical activity and accept full responsibility for my child’s participation. I release the instructor from any liability for injuries or health concerns that may arise.
