We are excited to have your child participate in our Chakradance program! As part of our community, we may take photos and videos during the classes and events for promotional purposes, including on our social media channels, website, and other marketing materials.
Please read and complete the form below to give consent for the use of your child’s image and participation in social media content.
I, the undersigned parent/guardian, consent to the use of my child’s image, video, or likeness in photographs, videos, or other digital media taken during the Chakradance classes or events. I understand that these materials may be used for promotional purposes on social media platforms, websites, and other marketing materials associated with the Chakradance program.
I understand that the content may be shared on platforms such as Facebook, Instagram, YouTube, or any other social media outlet related to the Chakradance program. The content will be used only for the purpose of promoting the program and its activities.
I agree that there will be no compensation provided for the use of my child’s image/video in any promotional materials or on social media platforms.
This consent will remain in effect for the duration of my child’s participation in the Chakradance program unless I revoke it in writing.
I acknowledge that I have read and understood this consent form, and I am providing my permission for my child’s participation in Chakradance classes and the potential use of their image/video for promotional purposes.
